About the Needs Assessment Survey

The targeted audience for this assessment (funded by the HRSA grant awarded to Illinois DHS) are individuals who have sustained an injury, family members, caregivers, and professionals working in the field of brain injury. It is for military and non-military personnel. You can provide assistance to those who may require it. The assessment doesn't ask for your name. All submitted information is anonymous, and no personal identifying information is required. This assessment does not impact on any services or benefits you may receive from the state or federal government. The results of this assessment are important because they will help the state identify the need for services and programs in the future.

The process is confidential and can be done on-line in about 5 minutes, or you can download a hard copy of the assessment and complete it manually. If you choose to complete the paper version of the assessment, the mailing address is listed at the end of the survey. The deadline for this survey is December 31, 2008. Our goal is to have this survey completed by as many people as possible within the state of Illinois so that we can further identify the need for services. The Brain Injury Association of Illinois will also be using the results of this assessment as we move forward with program development/growth and legislative activity.
